**Fx-5A**, also termed **5A-SM**, is an investigational synthetic high-density lipoprotein mimetic composed of the bihelical apolipoprotein A-I mimetic peptide 5A complexed with sphingomyelin. The peptide-lipid nanoparticle is designed to promote reverse cholesterol transport, principally by facilitating ATP-binding cassette transporter A1-mediated cellular cholesterol efflux. In preclinical studies, the sphingomyelin formulation increased cholesterol mobilization, enhanced ABCG1- and scavenger receptor class B member 1-mediated cholesterol efflux relative to a phosphatidylcholine formulation, reduced inflammatory cytokine release, and reduced atherosclerotic plaque burden in ApoE-deficient mice. Fx-5A has entered a Phase 1 intravenous study in healthy volunteers for cardiovascular-disease development.
