Overview

G-FLIP is a multi-drug chemotherapy regimen used primarily for pancreatic cancer treatment. It is a combination chemotherapy regimen consisting of five drugs: Gemcitabine, 5-Fluorouracil (5-FU), Leucovorin, Irinotecan, and Oxaliplatin (a platinum compound). This regimen was designed to incorporate multiple active single agents into a tolerable and effective combination for treating pancreatic cancer. ## Dosing and Administration The G-FLIP regimen is typically administered over two days in a specific sequence: **Day 1 Treatment:** - Gemcitabine: 500 mg/m² infused over 50 minutes - Leucovorin: 200-300 mg/m² infused over 30 minutes - Irinotecan: 80 mg/m² infused over 90 minutes - 5-FU: 1,500 mg/m² infused continuously over 20 hours **Day 2 Treatment:** - Oxaliplatin: 40 mg/m² infused over 2 hours The treatment is typically repeated every 14 days. ## Clinical Efficacy Studies have shown that G-FLIP produced objective responses in 7 out of 34 patients and stable disease in 6 out of 34 patients with pancreatic cancer. The overall median survival from diagnosis was reported as 15.5 months, with 28% of patients surviving at 2 years. G-FLIP has demonstrated effectiveness as a non-cross-resistant treatment for chemotherapy-refractory metastatic pancreatic cancer. It has also shown promise in patients who had failed previous chemotherapy regimens. ## Variations A modified version called G-FLIP-DM adds low doses of docetaxel and mitomycin C to the original G-FLIP regimen. This is typically administered to patients who show disease progression with G-FLIP treatment. Another variation combines G-FLIP with high-dose vitamin C (ascorbic acid), which is being studied to determine if it can enhance the effectiveness of the low-dose chemotherapy combination.
