Overview

**G-FLIP-DM** is a combination chemotherapy regimen used primarily for advanced pancreatic cancer. It consists of G-FLIP (low doses of Gemcitabine, Fluorouracil [5-FU], Leucovorin, Irinotecan, and Oxaliplatin) plus low doses of Docetaxel and Mitomycin C. This regimen is specifically designed as a second-line treatment for patients who experience disease progression while on G-FLIP therapy. The dosing of G-FLIP-DM follows a specific schedule where medications are administered on Days 1 and 2 of each 2-week treatment cycle. On Day 1, patients receive Gemcitabine (400 mg/m²) infused over 50 minutes, Leucovorin (200 mg/m²) infused over 30 minutes, Irinotecan (80 mg/m²) infused over 90 minutes, and 5-FU (1,500 mg/m²) infused continuously over 20 hours. On Day 2, patients receive Docetaxel (25 mg/m²) infused over 1 hour and Oxaliplatin (30 mg/m²). G-FLIP-DM works through multiple mechanisms targeting different aspects of cancer cell growth and division. Docetaxel works by encouraging cancer cell death, while Mitomycin C makes cancer cell DNA stick together, preventing cell division and tumor growth. The other components in the regimen also target various aspects of DNA replication and cell division. In clinical trials, G-FLIP-DM has been studied in combination with high-dose vitamin C (ascorbic acid) for patients with advanced pancreatic cancer. The primary endpoint in these studies was 12-month survival rate, with secondary endpoints including overall survival, quality of life, response rate, progression-free survival, and safety. The regimen is notable for using lower-than-standard doses of established chemotherapy drugs, which may help minimize toxic side effects while maintaining efficacy through synergistic drug interactions. G-FLIP-DM is considered a potential "salvage" or "rescue" regimen for patients who have failed first-line G-FLIP treatment.
