G17DT + cisplatin + 5-fluorouracil is an investigational combination regimen for advanced gastrointestinal cancers, particularly advanced gastric cancer. G17DT (also known as Gastrimmune or PAS) is a peptide-conjugate cancer vaccine that generates antibodies against gastrin-17, a hormone that stimulates tumor growth via the cholecystokinin-2 (CCK-B) receptor. The addition of cisplatin (a DNA crosslinking platinum compound) and 5-fluorouracil (a pyrimidine analog antimetabolite) forms a chemotherapeutic backbone where G17DT acts as immunotherapy by blocking gastrin-driven tumor proliferation, while cisplatin and 5-fluorouracil induce cytotoxicity and inhibit DNA and RNA synthesis, respectively. This combination has been studied in phase II and III clinical trials for stage IV gastric adenocarcinoma, and it has received orphan drug and fast track designations in multiple jurisdictions[1][2][3][4].
