This is a four-drug combination regimen consisting of obinutuzumab (GA101), dexamethasone, cytarabine, and cisplatin. Obinutuzumab is a type II glycoengineered anti-CD20 monoclonal antibody that targets the CD20 antigen on B lymphocytes, leading to cell death via direct cytotoxicity and immune-mediated mechanisms. Dexamethasone is a synthetic glucocorticoid corticosteroid with anti-inflammatory and immunosuppressant properties. Cytarabine is an antimetabolite chemotherapy agent that inhibits DNA synthesis by acting as a pyrimidine analog, primarily affecting rapidly dividing cells such as malignant lymphocytes. Cisplatin is a platinum-based chemotherapeutic agent that forms DNA crosslinks, inhibiting DNA replication and transcription, ultimately leading to apoptosis in cancer cells. This combination would be considered for the treatment of relapsed or refractory B-cell non-Hodgkin lymphoma or other aggressive lymphomas where similar regimens (such as R-DHAP) are used; however, in this case rituximab (R) has been replaced by obinutuzumab (GA101), which has shown higher response rates than rituximab in some studies[6]. The DHAP backbone (dexamethasone, high-dose cytarabine, cisplatin) is well-established for salvage therapy in lymphoma[1][2][3][4].
