Overview

Gabapentin + methylprednisolone is a combination therapy investigated by Jagiellonian University as a pre-emptive analgesic regimen for the management of postoperative pain, specifically in elderly patients undergoing total knee arthroplasty (TKA). Methylprednisolone is a synthetic glucocorticoid that reduces systemic inflammation and immune response by activating glucocorticoid receptors, thereby lowering levels of inflammatory markers like C-reactive protein (CRP). Gabapentin is an anticonvulsant that binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system, inhibiting the release of excitatory neurotransmitters involved in pain signaling. The combination aims to provide synergistic pain relief, reduce the requirement for postoperative opioids, and accelerate convalescence by targeting both inflammatory and neuropathic pain pathways before surgical trauma occurs.
