**Gabapentin + vitamin B complex** is a combination medication primarily used for the treatment of **neuropathic pain**, including painful diabetic neuropathy. The formulation usually contains gabapentin (an alpha-2 delta ligand that modulates calcium channels of nerve cells to decrease pain), and B-vitamins such as thiamine (B1) and cyanocobalamin or methylcobalamin (B12). The B vitamins contribute *neuroprotective* and *antiallodynic* effects, supporting nerve repair and reducing hyperalgesia. Combination therapy allows for lower gabapentin doses with similar efficacy and fewer adverse events such as dizziness and vertigo compared to gabapentin monotherapy. Mechanistically, gabapentin binds to the alpha-2 delta subunit of voltage-gated calcium channels, decreasing excitatory neurotransmitter release. B vitamins help regenerate damaged nerves and modulate neurotransmitter synthesis, serotonin, and GABA, attenuate glutamate, and act via the NO-cGMP pathway, hyperpolarizing nociceptor potassium channels and enhancing inhibitory control in the spinal cord[1][2][4][5][7].
