Gabexate is a synthetic small molecule serine protease inhibitor primarily indicated for the treatment of acute pancreatitis and disseminated intravascular coagulation. It acts by inhibiting several serine proteases, including trypsin, plasmin, plasma kallikrein, and thrombin, which are key enzymes in the inflammatory and coagulation pathways[2][4][8]. Gabexate also suppresses the production of inflammatory cytokines by attenuating NF-kappaB and c-Jun N-terminal kinase (JNK) pathway activity. This is achieved through prevention of IkappaB degradation, thereby deactivating NF-kappaB signaling[2][3][6]. The drug has been investigated for use in cancer, ischemia-reperfusion injury, sepsis-associated coagulopathy, and as an anticoagulant[1][5]. Gabexate mesilate is approved for clinical use in countries such as Italy and Japan[7].
