Gallium-68-DOTA-cetuximab is an investigational radioimmunoconjugate consisting of cetuximab—a chimeric IgG1 monoclonal antibody targeting the epidermal growth factor receptor (EGFR)—conjugated to the macrocyclic chelator DOTA (1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid) and radiolabeled with the positron-emitting radioisotope gallium-68. It is being investigated as a diagnostic PET imaging agent in a theranostic clinical trial led by Dr. Timothy Marr at Fremantle Hospital. The agent is designed to assess tumor EGFR expression and uptake in patients with advanced head and neck squamous cell carcinoma (HNSCC) who have exhausted standard surgical and radiotherapy options. Patients demonstrating sufficient tumor uptake of gallium-68-DOTA-cetuximab may then proceed to targeted radioimmunotherapy with therapeutic counterparts such as rhenium-188-DOTA-cetuximab or lutetium-177-DOTA-cetuximab.
