Gallium-68 NOTA hGZP is a first-in-human, peptide-based radiopharmaceutical developed as a PET imaging agent. It consists of a human granzyme B peptide (hGZP), which is conjugated with the macrocyclic chelator NOTA and radiolabeled with gallium-68 (^68Ga^). Its primary mechanism is targeting and binding to granzyme B, a serine protease released by activated cytotoxic lymphocytes, allowing for noninvasive PET imaging of immune activation in tumors. This agent is used to evaluate immune response, particularly to predict or monitor therapeutic response to immune checkpoint inhibitors (such as PD-1, PD-L1, CTLA-4, and LAG-3 inhibitors) in cancer. Its main indications studied to date include lymphoma and melanoma. The drug is in Phase 1 clinical trials for both indications[1][2][4][5][8].
