[68Ga]Ga-NI-FAPI-04 is an investigational radiopharmaceutical diagnostic agent developed by Peking Union Medical College Hospital for positron emission tomography (PET) imaging. It consists of a small-molecule fibroblast activation protein inhibitor (FAPI) derivative, NI-FAPI-04, chelated with the positron-emitting radioisotope Gallium-68. The agent specifically targets Fibroblast Activation Protein (FAP), a protease that is highly overexpressed on the surface of activated fibroblasts (myofibroblasts) in the tumor microenvironment and in various non-malignant fibrotic conditions. By binding to FAP with high affinity, [68Ga]Ga-NI-FAPI-04 allows for the non-invasive visualization and quantification of fibroblast activity across a broad spectrum of diseases, including solid tumors, cardiovascular diseases (such as hypertrophic cardiomyopathy and myocardial infarction), pulmonary fibrosis, and rheumatic immune diseases. It is designed to provide superior imaging contrast and pharmacokinetic properties compared to earlier FAPI tracers.
