Overview

Ga-68-NOTA-MSA is a radiopharmaceutical imaging agent designed for positron emission tomography (PET) imaging, specifically utilized in sentinel lymph node (SLN) mapping and lymphatic system evaluation. It consists of the radionuclide gallium-68 (Ga-68) chelated by 1,4,7-triazacyclononane-1,4,7-triacetic acid (NOTA) and conjugated to mannosylated human serum albumin (MSA). The mechanism of action involves the specific binding of the mannosyl moieties to the mannose receptor (CD206/MRC1) expressed on the surface of macrophages, particularly those residing in lymph nodes (sinusoidal macrophages) and Kupffer cells in the liver. Upon administration—typically via intradermal or subcutaneous injection near a tumor site—the tracer is transported via the lymphatic system to the sentinel lymph node, where it is internalized by macrophages, allowing for high-contrast PET/CT visualization. This agent offers several advantages over traditional Tc-99m labeled colloids, including faster clearance from the injection site, higher resolution imaging, and precise quantification, which aids in the surgical management of various malignancies such as breast cancer, cervical cancer, and melanoma.
