Gallium Ga 68 tilmanocept is a radiopharmaceutical diagnostic agent consisting of the synthetic molecule tilmanocept labeled with the positron-emitting radioisotope gallium-68. Tilmanocept is a mannosylated dextran compound that binds with high affinity to CD206 (macrophage mannose receptor), which is highly expressed on the surface of macrophages and dendritic cells, particularly in lymphatic tissue. When labeled with gallium-68, it enables targeted PET imaging for sentinel lymph node mapping and detection of metastatic spread in cancers such as thyroid carcinoma and endometrial carcinoma. The agent provides high specificity for sentinel nodes, facilitating image-guided tumor resection and lymphadenectomy. It has also been investigated for imaging atherosclerotic plaques due to its ability to target macrophage-rich tissues[1][2][5][7].
