**Galunisertib + gemcitabine** is a combination of two drugs, galunisertib and gemcitabine, investigated for the treatment of unresectable pancreatic cancer. Galunisertib is a first-in-class oral small-molecule inhibitor targeting the transforming growth factor-beta receptor I (TGF-βRI/ALK5) serine/threonine kinase, blocking TGF-β signaling, which is implicated in the progression and resistance of several cancers, including pancreatic cancer. Gemcitabine is a nucleoside analog and antimetabolite chemotherapeutic that inhibits DNA synthesis. The combination has shown improved overall survival and progression-free survival compared to gemcitabine alone in phase II clinical trials for unresectable pancreatic cancer, with a manageable toxicity profile[1][2][3][4].
