Overview
Ganciclovir is a synthetic nucleoside analogue of 2'-deoxyguanosine and a potent antiviral drug primarily used to treat cytomegalovirus (CMV) infections in immunocompromised patients, such as those with HIV/AIDS or organ transplant recipients. It is also indicated for herpetic keratitis of the eye and prevention of CMV disease in at-risk populations. Ganciclovir acts as a DNA polymerase inhibitor; after phosphorylation by viral thymidine kinase and subsequent cellular kinases to its active triphosphate form, it competitively inhibits viral DNA polymerase and incorporates into viral DNA. This results in premature chain termination and inhibition of viral replication. Its activity is highly selective for virus-infected cells due to the requirement for initial phosphorylation by virus-encoded enzymes[1][4][6][10].
