Overview

This combination therapy consists of the antiviral agent ganciclovir, the carbapenem antibiotic meropenem, and the glycopeptide antibiotic vancomycin. It is typically utilized in the management of critically ill patients, particularly those in pediatric or adult intensive care units presenting with severe sepsis, multiple organ dysfunction, or necrotizing pneumonia where broad-spectrum antimicrobial coverage is required. Ganciclovir is a nucleoside analog that inhibits viral DNA polymerase, providing activity against cytomegalovirus (CMV) reactivation, a common complication in immunocompromised or critically ill patients. Meropenem is a broad-spectrum carbapenem that targets Gram-negative and Gram-positive bacteria by inhibiting cell wall synthesis through binding to penicillin-binding proteins (PBPs). Vancomycin provides targeted coverage against Gram-positive organisms, including methicillin-resistant Staphylococcus aureus (MRSA), by binding to the D-alanyl-D-alanine portion of cell wall precursors, thereby inhibiting peptidoglycan polymerization.
