**Ganciclovir + sargramostim** is a combination regimen used primarily in patients with cytomegalovirus (CMV) infections, especially in immunocompromised individuals (such as those with advanced HIV/AIDS), to improve tolerance to ganciclovir therapy. Ganciclovir is an antiviral nucleoside analog that inhibits viral DNA polymerase, thereby preventing viral replication. Its main use is the treatment and prevention of CMV infections. One of the primary dose-limiting side effects of ganciclovir is neutropenia (low neutrophil count). Sargramostim is a recombinant human granulocyte-macrophage colony-stimulating factor (GM-CSF), which acts as a hematopoietic growth factor by stimulating proliferation and differentiation of bone marrow progenitor cells into granulocytes and macrophages. The use of sargramostim with ganciclovir helps to ameliorate or prevent ganciclovir-induced neutropenia, thereby allowing uninterrupted antiviral therapy and potentially delaying or preventing recurrence of CMV retinitis. This combination is not marketed as a fixed-dose product but is used together in clinical scenarios where management of ganciclovir-induced myelosuppression is necessary[1][3].
