Ganjianglingzhu decoction (GJLZD) is a traditional Chinese herbal formula developed by Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine, specifically indicated for the treatment of non-alcoholic fatty liver disease (NAFLD). Based on the classical Shenzhuo Tang from the *Synopsis of Prescriptions of the Golden Chamber* (*Jin Gui Yao Lue*), the decoction typically comprises four primary herbs: *Zingiberis Rhizoma* (Gan Jiang), *Poria* (Fu Ling), *Atractylodes macrocephala* (Bai Zhu), and *Glycyrrhizae Radix* (Gan Cao). GJLZD is designed to address the spleen-yang deficiency and dampness patterns often associated with metabolic disorders in TCM theory. Pharmacologically, it has been shown to modulate the gut-liver axis, improve insulin sensitivity, and reduce hepatic steatosis and inflammation. Its mechanism of action involves the activation of the AMPK/SIRT1 pathway and the regulation of lipid-related transcription factors such as PPAR-alpha, which collectively enhance fatty acid oxidation and inhibit lipogenesis in the liver.
