Garutadustat (formerly ISM5411) is an orally available, gut-restricted small molecule inhibitor of prolyl hydroxylase domain (PHD) enzymes. Developed by Insilico Medicine using its proprietary AI platform, Pharma.AI, the drug is designed to treat inflammatory bowel disease (IBD), specifically ulcerative colitis. Its mechanism involves the stabilization of hypoxia-inducible factor (HIF)-1α, which triggers the expression of genes responsible for intestinal barrier repair and anti-inflammatory responses. By being gut-restricted, garutadustat aims to provide localized therapeutic effects within the gastrointestinal tract while minimizing systemic exposure and the risk of systemic side effects like erythropoiesis. It is currently in Phase II clinical development.
