This drug refers to a four-drug combination regimen evaluated in the Phase 3 OFLOTUB clinical trial for the treatment of pulmonary tuberculosis. The regimen consists of gatifloxacin (a fourth-generation fluoroquinolone), rifampin, isoniazid, and pyrazinamide. It was designed as a 4-month shortened treatment course to improve patient compliance compared to the standard 6-month regimen. Gatifloxacin acts by inhibiting bacterial DNA gyrase and topoisomerase IV, while the other components target RNA synthesis, mycolic acid production, and membrane stability. Although the trial demonstrated that the regimen was safe and effective at initial bacterial clearance, it resulted in significantly higher relapse rates compared to standard care, leading to the conclusion that it should not replace the current 6-month standard of care for rifampin-sensitive tuberculosis.
