Gavocabtagene autoleucel is an investigational cell therapy consisting of autologous genetically engineered T cells that express a single-domain antibody targeting human mesothelin, fused to the CD3-epsilon subunit. This fusion allows the construct to be incorporated into the endogenous T cell receptor (TCR) complex, creating a T cell receptor fusion construct (TRuC). Upon administration, these modified T cells specifically recognize and bind to mesothelin-expressing tumor cells—mesothelin being a surface glycoprotein overexpressed in several cancers such as malignant mesothelioma, ovarian cancer, cholangiocarcinoma, and non-small cell lung cancer. The binding leads to activation of the engineered T cells and subsequent lysis of tumor cells through immunologic cytotoxicity. Compared with traditional CAR-T therapies, TRuC-T therapies like gavocabtagene autoleucel may result in less pro-inflammatory cytokine release while maintaining antitumor efficacy[1][2][3][4][5]. It is being developed and manufactured by Adaptimmune.
