GB104 is an orally administered investigational live biotherapeutic product composed of a lyophilized single bacterial strain, *Lactiplantibacillus plantarum* GB104, being developed by GI Biome and GI Longevity. It is being studied primarily in colorectal cancer and lower anterior resection syndrome after curative colectomy, where phase 1 clinical data showed favorable safety, tolerability, high compliance, and signals of improvement in bowel function. Preclinical work suggests GB104 has immunomodulatory and anti-tumor activity in syngeneic murine cancer models, while separate metabolic-disease studies indicate this strain can stimulate GLP-1 secretion, improve glucose control, limit weight gain, and reduce inflammatory macrophage polarization, supporting its classification as a microbiome-based therapeutic rather than a conventional small-molecule or biologic drug.
