GBFsen is a next-generation senolytic agent designed to selectively eliminate senescent cells while minimizing the systemic toxicities associated with broad-spectrum senolytics like navitoclax (ABT-263). It is a prodrug consisting of a galactose moiety conjugated to navitoclax, which remains inactive until it is specifically cleaved by **senescence-associated beta-galactosidase** (SA-β-gal), an enzyme highly overexpressed in senescent cells. Upon activation, the active navitoclax moiety is released, which then acts as a potent antagonist of the anti-apoptotic proteins **Bcl-2**, **Bcl-xL**, and **Bcl-w**. By inhibiting these survival factors, GBFsen triggers apoptosis specifically in senescent cells. This targeted approach is being investigated for the treatment of age-related pathologies, such as idiopathic pulmonary fibrosis (IPF), and as an adjuvant in cancer therapy to eliminate therapy-induced senescent cells that contribute to tumor recurrence and inflammation.
