GD2 CAR T cell therapy is an adoptive cellular immunotherapy involving T cells engineered to express a chimeric antigen receptor (CAR) specific for the disialoganglioside GD2. GD2 is a surface glycolipid highly expressed in various neuroectodermal and mesenchymal tumors, including neuroblastoma, osteosarcoma, and H3K27M-mutant diffuse midline gliomas (DMG), while having limited expression in normal tissues. Upon administration, these CAR T cells recognize and bind to GD2 on the surface of tumor cells, triggering T-cell activation, proliferation, and the release of cytotoxic granules (perforin and granzymes) and pro-inflammatory cytokines, ultimately leading to tumor cell lysis. Clinical development is particularly active in pediatric oncology, where GD2 CAR T cells have shown promising activity in DMG and neuroblastoma, though challenges such as T-cell exhaustion and the immunosuppressive tumor microenvironment remain significant hurdles.
