Gefarnate is a terpene fatty acid ester, specifically geranyl farnesylacetate, that serves as a mucosal protective agent. It is primarily used in the treatment of gastric and duodenal ulcers and chronic erosive gastritis. Gefarnate enhances the defense mechanisms of the gastric mucosa by increasing prostaglandin levels and the concentration of amino hexoses, thereby promoting mucosal healing and protecting against injury induced by nonsteroidal anti-inflammatory drugs (NSAIDs) or aspirin. In addition to its gastrointestinal applications, gefarnate is utilized in ophthalmology to treat dry eye syndrome by stimulating goblet cell repopulation and mucin-like glycoprotein secretion on the ocular surface. Emerging research also indicates that gefarnate may possess lipid-lowering properties by activating the SIRT1-AMPK-mTOR pathway, which induces autophagy and accelerates the degradation of triglycerides in hepatocytes.
