Gegen Qinlian Decoction (GQD) extract is a bioactive herbal extract derived from a classic Traditional Chinese Medicine (TCM) formula composed of *Puerariae Lobatae Radix*, *Scutellariae Radix*, *Coptidis Rhizoma*, and *Glycyrrhizae Radix et Rhizoma Praeparata cum Melle* in an 8:3:3:2 mass ratio. It contains key active constituents such as puerarin, berberine, baicalin, and glycyrrhizic acid. GQD extract exerts multi-target effects including anti-inflammatory activity (reducing IL-1β, TNF-α, and COX-2), activation of the Keap1/Nrf2 anti-oxidative pathway, and protection of the intestinal barrier. It is primarily investigated for the treatment of ulcerative colitis, type 2 diabetes, and as an adjunct in colorectal cancer therapy to enhance PD-1 blockade and reduce chemotherapy-induced toxicity. Recent research highlights the role of self-assembled natural nanoparticles (GQD-Nnps) within the extract that enhance the colonic absorption and hepatic distribution of its active components.
