GELAD is a chemotherapy regimen used primarily for the treatment of early-stage natural killer/T-cell lymphoma (NK/TCL). The regimen consists of gemcitabine, etoposide, pegaspargase, and dexamethasone administered in a specific schedule. It is often combined with sandwiched radiotherapy (intensity-modulated radiotherapy) to improve outcomes. The mechanism involves cytotoxic effects from gemcitabine and etoposide as chemotherapeutic agents, pegaspargase which depletes asparagine affecting lymphoma cells, and dexamethasone as an anti-inflammatory corticosteroid that also has lympholytic properties. This combination has shown high overall response rates and complete response rates in clinical studies for NK/TCL patients. The regimen was studied in a clinical trial registered under NCT02733458 with promising survival outcomes and manageable toxicity profiles.[4]
