The gelatin sponge provides a physical scaffold for hemostasis, while thrombin acts as a topical coagulant by converting fibrinogen into fibrin. Tobramycin, an aminoglycoside antibiotic, is included to provide broad-spectrum antibacterial prophylaxis against surgical site infections. Dexamethasone, a potent corticosteroid, is utilized to suppress the postoperative inflammatory response and minimize the formation of granulation tissue and membranous adhesions at the anastomosis site, which are the primary causes of surgical failure. In clinical trial settings, this specific packing protocol serves as a standard-of-care control to evaluate the safety and efficacy of newer packing materials like hydroxybutyl chitosan.
