GEM 21S (Growth-factor Enhanced Matrix) is a synthetic bone grafting system and tissue engineering product indicated for the treatment of periodontal bone defects and ridge augmentation. It is a combination product consisting of two primary components: a highly purified recombinant human platelet-derived growth factor-BB (rhPDGF-BB), also known as becaplermin, and an osteoconductive scaffold of beta-tricalcium phosphate (β-TCP). The rhPDGF-BB acts as a potent mitogen and chemotactic agent for mesenchymal cells, including osteoblasts and periodontal ligament fibroblasts, stimulating their migration and proliferation to promote the regeneration of alveolar bone, cementum, and periodontal ligament. The β-TCP matrix provides a structural framework that supports new bone formation. Originally developed by BioMimetic Therapeutics and currently manufactured by American Regent (formerly Luitpold Pharmaceuticals), GEM 21S has been evaluated in clinical trials at the University of Alabama at Birmingham for lateral ridge augmentation in patients with deficient bone ridges prior to dental implant placement.
