Gemcabene is a small molecule, orally administered dialkyl ether dicarboxylic acid with antihyperlipidemic and anti-inflammatory activity. It lowers low-density lipoprotein cholesterol (LDL-C), decreases triglycerides, and raises high-density lipoprotein cholesterol (HDL-C). Gemcabene acts through multiple mechanisms including inhibition of hepatic de novo cholesterol and triglyceride synthesis, downregulation of apolipoprotein C-III expression to enhance very low density lipoprotein clearance, reduction of C-reactive protein gene expression, and modulation of inflammatory pathways. It has been investigated for the treatment of hypercholesterolemia, hypertriglyceridemia, non-alcoholic fatty liver disease (NAFLD), non-alcoholic steatohepatitis (NASH), atherosclerosis, cardiovascular disorders, hypertension, osteoarthritis, and other metabolic diseases. The drug was originally developed by Pfizer and later by companies such as Gemphire Therapeutics (now MetaVia) and Emory University[1][2][4][5][7].
