Overview

Gemcitabine + nab-paclitaxel is a combination chemotherapy regimen primarily used for the treatment of advanced or metastatic pancreatic adenocarcinoma. Gemcitabine (Gemzar) is a nucleoside analog that inhibits DNA synthesis, leading to cell death in rapidly dividing cells. Nab-paclitaxel (Abraxane) is a nanoparticle albumin-bound formulation of paclitaxel, which stabilizes microtubules and prevents their disassembly, thereby inhibiting cell division. Nab-paclitaxel exploits albumin-mediated transport mechanisms to enhance drug delivery to tumors. The combination has demonstrated improved overall survival compared to gemcitabine alone in clinical trials for pancreatic cancer. Mechanistically, preclinical studies suggest that nab-paclitaxel may increase intratumoral concentrations of gemcitabine by reducing levels of cytidine deaminase, the enzyme responsible for metabolizing gemcitabine[3][4][6].
