Gemcitabine + busulfan + melphalan (Gem-Bu-Mel) is a high-dose chemotherapy conditioning regimen developed by the M.D. Anderson Cancer Center for patients with relapsed or refractory Hodgkin's disease (Hodgkin lymphoma) undergoing autologous hematopoietic stem cell transplantation (AHCT). The regimen utilizes the synergistic effects of gemcitabine, a nucleoside analog that inhibits DNA synthesis and repair, with the alkylating agents busulfan and melphalan. Gemcitabine is known to enhance the cytotoxicity of alkylators by inhibiting the repair of DNA interstrand cross-links. This combination is designed to provide intensive cytoreduction to overcome chemoresistance in heavily pretreated patients before they receive stem cell support.
