This combination regimen, consisting of gemcitabine, clofarabine, and busulfan (Gem/Clo/Bu), is a myeloablative conditioning treatment developed by the M.D. Anderson Cancer Center for patients undergoing allogeneic hematopoietic stem cell transplantation for refractory lymphomas. Gemcitabine and clofarabine are nucleoside analogs that inhibit DNA polymerase and ribonucleotide reductase, which not only provides direct antineoplastic activity but also inhibits DNA repair mechanisms. This inhibition sensitizes the malignant cells to the DNA-alkylating effects of busulfan, creating a synergistic cytotoxic effect. The regimen is specifically designed to treat aggressive and refractory forms of B-cell non-Hodgkin lymphoma, T-cell non-Hodgkin lymphoma, and Hodgkin lymphoma, aiming to maximize tumor cytoreduction and facilitate successful donor engraftment.
