This therapeutic regimen is a multi-agent protocol developed and studied by the Benaroya Research Institute for the treatment of borderline resectable or locally advanced pancreatic adenocarcinoma. The protocol typically involves induction chemotherapy using gemcitabine and docetaxel, followed by a chemoradiation phase consisting of oxaliplatin, 5-fluorouracil (5-FU), and interferon alfa-2b, often culminating in surgical resection (pancreaticoduodenectomy). Gemcitabine and 5-FU are antimetabolites that inhibit DNA synthesis, while docetaxel is a taxane that stabilizes microtubules to arrest cell division. Oxaliplatin acts as a platinum-based DNA alkylating agent that induces lethal cross-links. Interferon alfa-2b is utilized as a biological response modifier to enhance the cytotoxic and radiosensitizing effects of the chemotherapy. This intensive multi-modality approach is designed to downstage tumors and improve the likelihood of achieving curative-intent surgical outcomes.
