Combination chemotherapy of three small-molecule cytotoxic agents used investigationally and off‑protocol in oncology. Gemcitabine is a deoxycytidine analog that inhibits DNA synthesis through diphosphate-mediated ribonucleotide reductase inhibition and triphosphate incorporation causing chain termination. Doxorubicin is an anthracycline that intercalates DNA, inhibits topoisomerase II, and generates free radicals. Paclitaxel is a taxane that stabilizes microtubules and arrests cells in mitosis. This triplet has been studied primarily as an anthracycline–taxane backbone augmented by gemcitabine in metastatic breast cancer and cisplatin‑ineligible or renally impaired urothelial carcinoma, often on a biweekly schedule with growth factor support, showing activity with myelosuppression as a key toxicity.[1][7][3]
