Overview

The combination of gemcitabine, ifosfamide, cisplatin, and granulocyte colony-stimulating factor (G-CSF) is a multi-agent chemotherapy regimen primarily used as salvage therapy for relapsed or refractory germ cell tumors (notably testicular cancer) and has also been studied in platinum-resistant ovarian cancer. Gemcitabine is a nucleoside analog that inhibits DNA synthesis; ifosfamide is an alkylating agent causing DNA cross-linking; cisplatin forms DNA adducts leading to apoptosis; and G-CSF stimulates neutrophil production to reduce the risk of severe neutropenia associated with intensive chemotherapy. The regimen exploits the synergistic cytotoxic effects of these agents while using G-CSF to support bone marrow recovery[1][2][3][5]. This combination has demonstrated high response rates in relapsed germ cell tumors and moderate efficacy in platinum-resistant ovarian cancers, though it carries significant risk for myelosuppression[1][2].
