Gemcitabine + ifosfamide + paclitaxel is a combination chemotherapy regimen composed of three cytotoxic agents—gemcitabine, ifosfamide, and paclitaxel. Each drug has a distinct mechanism of action: - Gemcitabine is a nucleoside analog that inhibits DNA synthesis by incorporating into DNA and causing chain termination. - Ifosfamide is an alkylating agent that crosslinks DNA strands, leading to cell death. - Paclitaxel stabilizes microtubules and prevents their depolymerization, thereby inhibiting mitosis. This combination has been investigated as a platinum-free alternative for advanced or metastatic non-small cell lung cancer (NSCLC) and as salvage therapy in relapsed germ cell tumors after failure of first-line cisplatin-based regimens. Clinical studies have shown the regimen to be active with manageable toxicity profiles in these settings[1][2]. The combination aims to leverage the synergistic effects of these agents while providing options for patients who may not tolerate platinum-based therapies.
