Overview

Gemcitabine + Jelmyto is a combination regimen involving sequential or concurrent administration of two chemotherapeutic agents for the treatment of urothelial cancers, particularly upper tract urothelial carcinoma (UTUC). Gemcitabine is a nucleoside analog that inhibits DNA synthesis, leading to cell death in rapidly dividing tumor cells. Jelmyto is a proprietary reverse thermal gel formulation containing mitomycin, which becomes semi-solid at body temperature to allow prolonged local exposure; mitomycin acts as an alkylating agent that crosslinks DNA and inhibits cancer cell proliferation. This combination aims to maximize local tumor control while preserving kidney function in patients with low-grade UTUC who are not candidates for surgery or wish to avoid nephroureterectomy[1][2][3]. The approach leverages the established efficacy of both agents—gemcitabine’s systemic cytotoxicity and Jelmyto’s chemoablative effect—to potentially improve outcomes over monotherapy.
