Overview

Gemcitabine + nab-paclitaxel + capecitabine is a combination chemotherapy regimen used primarily in the treatment of pancreatic adenocarcinoma, particularly in advanced, unresectable, or borderline resectable cases. This regimen combines three agents with distinct mechanisms of action: - **Gemcitabine** is a nucleoside analog that inhibits DNA synthesis by incorporating into DNA and causing chain termination. - **Nab-paclitaxel** (nanoparticle albumin-bound paclitaxel) stabilizes microtubules and prevents their disassembly, thereby inhibiting cell division. - **Capecitabine** is an oral prodrug converted to 5-fluorouracil (5-FU), which inhibits thymidylate synthase and disrupts DNA synthesis. The combination aims to exploit synergistic effects for improved efficacy over single-agent therapy. Clinical trials have explored this regimen for first-line treatment in metastatic or locally advanced pancreatic cancer. The addition of nab-paclitaxel to gemcitabine has shown improved survival compared to gemcitabine alone; adding capecitabine may further enhance response rates but can increase toxicity[1][3][8].
