Overview

This neoadjuvant treatment regimen is being evaluated by the University of Florida in the GAIN-1 Phase II clinical trial for patients with resectable or borderline-resectable pancreatic adenocarcinoma. The regimen consists of systemic chemotherapy using gemcitabine and nab-paclitaxel (Abraxane), followed by chemoradiotherapy (radiation therapy combined with chemotherapy) for high-risk or borderline-resectable cases prior to surgical resection. Gemcitabine is a nucleoside analog that inhibits DNA synthesis, while nab-paclitaxel is an albumin-bound nanoparticle formulation of paclitaxel that disrupts microtubule dynamics. The addition of radiation therapy aims to improve local control and increase the likelihood of achieving negative surgical margins (R0 resection).
