Overview

This is a combination therapy that includes a chemotherapy regimen (gemcitabine + oxaliplatin, also known as GEMOX), an anti-angiogenic agent (anlotinib), and an immune checkpoint inhibitor (sintilimab). The combination has been studied primarily for biliary tract cancer (BTC) and other advanced malignancies. The therapy combines multiple mechanisms of action: gemcitabine and oxaliplatin provide cytotoxic effects against cancer cells, anlotinib inhibits angiogenesis by targeting VEGFR and other kinases, and sintilimab blocks the PD-1 pathway to enhance immune response against tumors. Clinical trials have shown promising efficacy and manageable safety profiles for this combination in treating advanced biliary tract cancers. In particular, the addition of anlotinib and sintilimab to gemcitabine-based chemotherapy has demonstrated improved progression-free survival (PFS) and objective response rates (ORR) compared to chemotherapy alone. The combination has been studied in both first-line and second-line settings for various cancers including biliary tract cancer, non-small cell lung cancer (NSCLC), and small cell lung cancer (SCLC). Side effects appear to be manageable, with the most common adverse events being related to the individual components of the regimen. This multi-modal approach represents an emerging treatment strategy that combines traditional chemotherapy with targeted therapy and immunotherapy to potentially improve outcomes in difficult-to-treat cancers.
