Overview

Gemcitabine + oxaliplatin + paclitaxel is a multi-agent chemotherapy regimen combining three small-molecule cytotoxics: the antimetabolite gemcitabine, the platinum compound oxaliplatin, and the taxane paclitaxel. Gemcitabine is phosphorylated intracellularly and inhibits ribonucleotide reductase and DNA polymerase, causing DNA synthesis arrest and apoptosis. Oxaliplatin forms DNA crosslinks leading to DNA damage and cell death. Paclitaxel stabilizes microtubules and prevents depolymerization, blocking mitosis. This combination, sometimes referred to as GemPOx, has been investigated as an induction regimen for relapsed or recurrent intracranial germ cell tumors, with plans for consolidation and autologous stem cell rescue, and represents a broader cytotoxic strategy leveraging non-overlapping mechanisms of action of an antimetabolite, a platinum, and a taxane[6]. Sequential and alternating use of gemcitabine–oxaliplatin with a taxane-containing regimen has also been explored in pancreatic ductal adenocarcinoma, reflecting clinical interest in integrating these agents in pancreatic cancer treatment paradigms[2]. Gemcitabine–oxaliplatin (GemOx) and gemcitabine–paclitaxel dyads have established activity in several solid tumors, supporting the rationale for the triplet in investigational settings[1][3].
