Overview

This combination regimen consists of the chemotherapy agents gemcitabine, paclitaxel, and doxorubicin, supported by the bone marrow stimulant pegfilgrastim. Gemcitabine is a nucleoside analog that inhibits DNA synthesis by inhibiting ribonucleoside reductase and incorporating into DNA. Paclitaxel is a taxane that stabilizes microtubules, thereby preventing cell division. Doxorubicin is an anthracycline that intercalates DNA and inhibits topoisomerase II, leading to DNA damage and apoptosis. Pegfilgrastim is a long-acting form of granulocyte colony-stimulating factor (G-CSF) used to reduce the incidence of neutropenia associated with myelosuppressive chemotherapy. This specific combination was evaluated by the M.D. Anderson Cancer Center in a Phase II clinical trial for patients with metastatic transitional cell carcinoma (TCC) of the urothelium, particularly those with renal insufficiency who may not tolerate standard cisplatin-based regimens.
