The GPED regimen is a multi-agent chemotherapy protocol consisting of gemcitabine, pegaspargase, etoposide, and dexamethasone. It is primarily developed and investigated by Beijing Tongren Hospital for the treatment of relapsed/refractory or advanced extranodal NK/T-cell lymphoma (ENKTCL), including cases complicated by hemophagocytic lymphohistiocytosis. Gemcitabine is a nucleoside analog that inhibits DNA synthesis by targeting ribonucleoside reductase and incorporating into DNA; pegaspargase is a pegylated form of L-asparaginase that depletes serum asparagine, an amino acid essential for the survival of lymphoma cells; etoposide is a topoisomerase II inhibitor that induces DNA strand breaks; and dexamethasone is a corticosteroid that provides lympholytic and anti-inflammatory effects. This combination therapy is designed to overcome the resistance often seen in advanced NK/T-cell lymphomas by utilizing multiple mechanisms of action.
