Overview

Gemcitabine + simvastatin is an investigational combination therapy consisting of gemcitabine, a nucleoside analog chemotherapeutic agent, and simvastatin, a statin class lipid-lowering drug. Gemcitabine acts as a cytidine analog that inhibits DNA synthesis by incorporating into DNA during replication, leading to cell cycle arrest and apoptosis in rapidly dividing tumor cells. Simvastatin inhibits HMG-CoA reductase, the rate-limiting enzyme in cholesterol biosynthesis; it also exhibits potential antineoplastic effects through induction of apoptosis and inhibition of cell proliferation. The rationale for combining these agents is based on preclinical evidence suggesting synergistic antitumor activity. This combination has been studied primarily in advanced pancreatic cancer but has not demonstrated significant clinical benefit over gemcitabine alone in phase II trials[1][4][6].
