Gemcitabine Rompharm is a generic version of the antineoplastic agent gemcitabine, developed by Rompharm. It is a pyrimidine nucleoside analog that functions as an antimetabolite. Once transported into cells, gemcitabine is phosphorylated into its active forms: gemcitabine diphosphate (dFdCDP) and gemcitabine triphosphate (dFdCTP). dFdCDP inhibits ribonucleotide reductase, the enzyme responsible for producing deoxynucleotides for DNA synthesis, thereby depleting intracellular nucleotide pools. dFdCTP competes with deoxycytidine triphosphate for incorporation into DNA; once incorporated, it allows for the addition of only one additional nucleotide before DNA polymerase is unable to proceed, a process known as 'masked chain termination' that evades DNA repair mechanisms. It is approved for the treatment of pancreatic cancer, non-small cell lung cancer (NSCLC), and bladder cancer.
