Overview

This intervention refers to a multimodal analgesic approach combining general anesthesia with regional nerve blocks—specifically the Pericapsular Nerve Group (PENG) block and the Lumbar Erector Spinae Plane (ESP) block—using bupivacaine. Developed and evaluated by Tanta University, this combination is primarily indicated for postoperative pain management in geriatric patients undergoing total hip arthroplasty (THA). Bupivacaine is a long-acting local anesthetic that functions as a sodium channel blocker, preventing the generation and conduction of nerve impulses to provide localized analgesia. The PENG block targets the articular branches of the femoral, obturator, and accessory obturator nerves to provide hip-specific pain relief, while the ESP block offers broader truncal analgesia. This combination aims to reduce postoperative opioid consumption and facilitate early mobilization in elderly surgical populations.
