The Genetic-Tailored AntiThrombotic Strategy is a personalized therapeutic regimen developed by the Texas Cardiac Arrhythmia Research Foundation for the prevention of device-related thrombosis (DRT) in patients with atrial fibrillation undergoing percutaneous left atrial appendage occlusion (LAAO). Evaluated in the FADE-DRT Phase 4 trial, the strategy involves the administration of aspirin in combination with either clopidogrel or a half-dose novel oral anticoagulant (NOAC), with the choice of the second agent determined by the patient's CYP2C19 genotype. This approach aims to optimize the balance between thrombotic prevention and bleeding risk by accounting for genetic variations that affect the metabolism and efficacy of antiplatelet therapy.
