Drug intelligence / Profile preview
This appears to be a combination therapy used primarily in surgical settings, particularly for preventing or treating infections in appendicitis cases and other surgical procedures. The search results indicate that gentamicin and metronidazole are typically administered systemically (intravenously or orally), while povidone-iodine is applied topically as an antiseptic solution. ## Therapeutic Use The combination is used in surgical settings, particularly for: 1. Prevention of surgical site infections 2. Treatment of peritonitis secondary to hollow viscus perforation 3. Management of appendicitis (both early and late stages) In one study, patients with appendicitis were randomized into two groups - one receiving pre-operative systemic gentamicin and metronidazole, while the other received the same antibiotics plus 1% topical povidone-iodine solution. For early appendicitis cases, only one dose of antibiotics was used, while for late appendicitis (gangrenous and perforated), antibiotics were continued for 7 days. ## Efficacy and Safety Interestingly, the research shows mixed results regarding the efficacy of adding povidone-iodine to the antibiotic regimen: * For early appendicitis, postoperative wound sepsis rates were very low in both groups with no statistical difference. * For late appendicitis (gangrenous and perforated), patients who received the topical povidone-iodine actually had a significantly higher wound infection rate (8 out of 51) compared to those who received only antibiotics (1 out of 52). The study concluded that 1% povidone-iodine adversely affects the wound infection rate in late appendicitis and should not be used in these cases. ## Components 1. **Gentamicin**: An aminoglycoside antibiotic effective against many gram-negative bacteria. 2. **Metronidazole**: An antibiotic effective against anaerobic bacteria and certain parasites. 3. **Povidone-iodine**: A topical antiseptic used for skin disinfection before and during surgical procedures. This combination represents a multi-modal approach to infection prevention and treatment, targeting different types of microorganisms through different mechanisms and routes of administration.
