Gentamicin + clindamycin is an investigational combination antibiotic regimen primarily studied for the prophylaxis of acute post-traumatic bacterial endophthalmitis (APBE) following penetrating eye injuries. Developed by the Shahid Beheshti University of Medical Sciences, this combination leverages the synergistic effects of two distinct classes of antibiotics: gentamicin, an aminoglycoside that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, and clindamycin, a lincosamide that targets the 50S ribosomal subunit. In clinical trials (notably NCT00398658), the regimen was administered via intracameral or intravitreal injection immediately following primary surgical repair of eye injuries. The combination aims to provide broad-spectrum coverage against common ocular pathogens, significantly reducing the incidence of post-traumatic infection compared to standard systemic and topical prophylaxis alone.
